Form Design allows you to design forms to gather data that can then be used to provide actionable insights about your business and shape the strategic goals of the organisation. In the platform, forms can be used to gather and provide information to employees. You can also duplicate any form design created if you would like to create a slightly modified version of an already existing form.

To Create a New Form Design
1. Navigate to Engagement >> Form Designs
2. Click Create New Form Design
3. You will then be prompted to configure the Form Design Settings. The following details are customisable:
- Form Name: Visible to respondents and used across the system to identify forms.
- Category: A Form Design can be assigned to the appropriate category (i.e. Onboarding/ Training) or left as ‘uncategorised’. You can customise the list of categories to suit your business through the Settings menu.
- Type: Most form designs will relate to a person’s job. Some will be training-related. You can select the type here.
- Respondent Type: Determines who is to complete the form (i.e. an employee, their supervisor, a specific person or an email address). Please note if configured to send to an email address that is not listed within intelliHR but the respondent will need to log in before completing the form.
- Description: A brief note about the form design. This is not visible to respondents and is purely for your use.
- Completion Recipient Actions: Whenever that form is completed, you may elect for a copy of the responses to be automatically sent to one or more people.
- Trigger on Completion: Whenever that form is completed, you may elect for a new form to be created and assigned in the system. This new form will behave according to its own Form Design Settings. You can link multiple form designs together with triggers.
- Due Timeframe: This section specifies the number of business days before the form is due to be completed. A zero specifies that the form is due immediately after it has been created.
- Reminder For Respondent: The number of business days after the issue date of the form that an email reminder will be sent to the respondent.
- Escalation to Manager: If a respondent has not completed a form within a certain time frame, you may elect to have that form automatically escalated to that person’s direct manager.
- Auto-Ignore: To help keep your form list up to date, you may wish to set a time frame for the automatic archiving of incomplete forms.
4. Add and edit fields to the new form design as needed (see Adding Fields to a Form Design).
5. Select Preview to view your new Form Design.
To Duplicate a Form Design
1. Navigate to Engagement >> Form Designs.
2. Select Duplicate on the form design you wish to duplicate. There will now be two identical copies of that form design in your library.
3. Change the name of the new copy in the settings page for that form design.
4. Add and edit Fields as required.
5. Select Preview to view your new Form Design.
