The Declaration form is used to get a confirmation from an employee before they clock out of their shift. The text they must acknowledge is customisable and can be displayed on the Humanforce Kiosk and Mobile App.
Set up
You will need to ensure that the Declaration Form is enabled and the system settings are updated.
1. Click on Admin > Clock In Settings. Click Edit for the clock in setting(s) that you would like to add the form to.
2. On the Other Options tab, tick the box for Declaration form in Kiosk and App.
The Hours field determines how long a rostered shift must be before the declaration form will appear. Whenever a shift goes over the number of hours in this field, the declaration form will appear during clock off. If you'd like the form to appear for all shifts, we recommend using a small number like 1. If you only want it to appear when a shift goes into overtime, then it might be a larger number like 9.
The Non-Rostered Min. Hours field determines how long a non-rostered shift must be before the declaration form will appear. Whenever a shift goes over the number of hours in this field, the declaration form will appear during clock off. If you'd like the form to appear for all shifts, we recommend using a small number like 1. If you only want it to appear when a shift goes into overtime, then it might be a larger number like 9.
The Manager field is usually left blank. If a name is selected, that person will be the only one who can add manager consent, which can only be done in the Back Office and with a finger vein scanner.
The Header Text and Footer Text is the message the employee will see and need to acknowledge before clocking out. As the text will appear in the same paragraph, it's recommended to just use one text box.
3. In Admin > System Settings, go to the Clocking tab. There is an option to Block clock-out if overtime consent fails. Tick this box and Save Changes at the bottom of the screen.

If everything has been set up correctly, when an employees clocks out for a shift that would cause them to exceed the number of hours set up in the Clock In Settings, they will receive the following consent form on the kiosk:
